If you’re the surviving spouse or child of a service member who died while on active duty, your military casualty assistance officer will help you to complete an Application for DIC, Death Pension, and/or Accrued Benefits by a Surviving Spouse or Child. The officer will help you mail the form to the correct VA regional office.

Government Medallion. Did you know you can get a beautiful, free, Military Medallion for a headstone or marker for a crypt in a private or local Military cemetery? You can choose a Navy, Army, Marines or Coast Guard designation. Just download VA Form VA40-1330M from Google or the VA website. The Medallion is made of bronze and is available in three sizes: large, medium, and small.
The eligibility requirements are: Any deceased Veteran discharged under honorable conditions, who served in the Armed Forces on or after April 6, 1917 and is buried in a private or local Government cemetery in a grave marked with a privately purchased headstone or marker. Please attach a copy of the deceased Veteran's discharge certificate (DD Form 214 or equivalent) or a copy of other official document(s) establishing qualifying military service. Do not send original documents; they will Not be returned. For more information regarding medallion Eligibility, affixing procedures, or sizes, visit their website at Burials and memorials. For assistance in obtaining Veterans’ records please call your local Department of Veterans Affairs Regional Office at 1-800-827-1000.
An applicant for a Medallion may be any of the following: A decedent's family member, which includes the decedent's spouse or individual who was in a legal union as defined in 38 CFR 3.1702(b)(1)(ii) with the decedent; a child, parent, or sibling of the decedent, whether biological, adopted, or step relation; and any lineal or collateral descendant of the decedent. It can also be a personal representative, defined as a family member or other individual who has identified himself or herself as the person responsible for making decisions concerning the interment of the remains of or memorialization of a deceased veteran.
